This is a Bulk Mail Request Form. Print this out, and send it through Mail Services, to P&D for a Bulk Mail pick-up, or you may obtain several forms from Mail Services. Utilizing this form will ensure that your bulk mailing will be picked up in a timely manner. As stated on the form, the form should ARRIVE at Printing and Duplicating at least one day in advance of the date that the pickup is required.
