Address/Name Change
Address changes can be done through our office, online (OneStart), or by fax. Please include your name, your ten digit identification number, old address and current address along with a daytime phone number. It can be faxed to 812-941-2351 or sent to:

To file a name change you must come into the Registrar's Office to complete a name change form. Please be ready to provide picture identification, marriage license, or other legal document ( i.e. divorce, adoption...) with your correct legal name.
