Academic Success Center Suspension Policy

Deadline for an Appeal for Reinstatement is Jan. 4th 2007 at 5pm.

To file an appeal, you must return your typed responses to the “Academic Success Center Appeal for Reinstatement” form to our office.  You may complete this form in one of two ways:

1. Type your responses directly onto the form and print (make sure to clear your information off the page once you  have printed).
2. Or, print the blank form and attach your typed responses.
