Student Application Reference Form

Reference forms are to be completed by an instructor, employer, mentor, coach, or other individual capable of speaking to the student’s strengths.  These individuals may certainly include a letter of recommendation, but the completed form must accompany the letter.  Reference materials may be submitted via email to iushp@ius.edu, via fax to 812-941-2170, or via postal mail to the following address:
