Lost or Stolen IDs
If you suspect your U Card to be lost or stolen, and to avoid fraudulent charges to your account, you should contact Campus Card Services immediately at 812-941-2335 or email us at ucard@ius.edu during business hours so your card can be deactivated. After hours you should contact the Campus Police at 812-941-2400.
If you suspect that your card has been stolen, please file a police report with the Campus Police. If you show a copy of the police report to Campus Cards Services, there will not be a card replacement fee.
If you've lost your U Card, simply bring a photo ID and $25 to receive a replacement card.
