Lost or Stolen IDs
If you suspect your UCard to be lost or stolen, and to avoid fraudulent charges to your account, you should deactivate your card immediately, contact Campus Card Services immediately at 812-941-2335 or email us at email@example.com during business hours.
To deactivate your UCard, go to Transaction History. Under the "Cardholder Information" section click the "Lost or stolen UCard" link to enable the deactivation.
If you suspect that your card has been stolen, please file a police report with the Campus Police.
If you've lost your UCard, simply bring a photo ID and $25 to receive a replacement card.
If you have been issued a temporary security access card, you must present the temporary card to Campus Card Services before a new UCard can be issued. If you misplace the temporary card, there will be an additional $25.00 replacement fee.
